EINLEITUNG: Die Cochlea Implantation von Patienten mit Hochtonh?rverlust jedoch erhaltenem H?rverm?gen im Tieftonbereich ist in den letzten Jahren zu einer neuem Standard der Versorgung geworden. Das Ziel dieser Studie war es, die Rate des Restgeh?rerhalts bei Patienten, die sich einer Cochlea Implantation für die elektrisch-akustische Stimulation per suprameatalem Zugang unterzogen, zu ermitteln. METHODE: 5 Patienten (mittleres Alter 48,2 Jahre), die mit Cochlea Implantaten der Firma MedEl (Combi 40+, Pulsar) und unterschiedlichen Elektroden (Custom made, Flex soft, Flex EAS) versorgt wurden, wurden bezüglich Restgeh?rerhalt nach einem mittleren Nachbeobachtungszeitraum von 35,6 Monaten (24 bis 77 Monate) untersucht. RESULTATE: 3 Patienten zeigten einen partiellen H?rerhalt, bei 2 Patienten wurde ein kompletter Verlust des Restgeh?rs beobachtet. SCHLUSSFOLGERUNG: Obwohl die Rate des Restgeh?rerhalts in der vorliegenden Patientengruppe, die mittels suprameatalem Zugang operiert wurde, geringer ist, als bei jenen Patienten, die einen konventionellen Mastoidektomie-Zugang erhielten, so zeigt diese Studie jedoch erstmalig, da? es in der Tat m?glich ist, das Restgeh?r auch bei einem Zugang ohne Mastoidektomie zu erhalten. Dennoch, um die Frage zu beantworten, ob diese Technik grunds?tzlich bei Patienten mit elektrisch-akustischer Stimulation empfohlen werden kann, ist weitere Erfahrung notwendig.

Objectives/Hypothesis: Recent in vitro models simulating biofilm formation on medical polymers are restricted to only short-term observation periods of 2 hours to 12 days. Study Design: The goal of this study was to develop an in vitro model to generate a long-term polymicrobial biofilm with Candida albicans (C albicans) and Streptococcus salivarius (S salivarius) on thermoplastic medical grade silicone (TPS) and polyurethane (TPU) and continuous documentation of growth kinetics. Methods: Test platelets of TPS and TPU were incubated in well plates in RPMI agar at 37°C. Both microbial specimen were isolated from explanted voice prostheses and added every second day for 28 days. Afterward, only the nutrition solution has been changed regularly. Biofilm kinetics were monitored using a specially designed image analysis software to calculate the percentual surface covering of each platelet. Biofilm architecture was investigated by scanning electron microscopy (SEM). Microbial infiltration was examined by crystal violet staining and thin section microscopy. Results: On both materials tested, a cover of living candida biofilm could be generated over 140 days. Colonization was permanent with at least 10% surface coverage. Initially, both materials showed coverage of up to 80% followed by biofilm detachment, which could be reduced by adding planktonic microbes. SEM confirmed three-dimensional biofilm architecture with dimorphic candida growth. Microbial material infiltration of nonhypheal types was proved in 2 TPU platelets, but not in TPS. Conclusions: The in vitro model presented in this study mimics in vivo events of biofilm formation on medical polymers with continuous monitoring of living biofilm kinetics. Laryngoscope, 2013

Objectives: Similar to other zona pellucida mutations in the alpha-tectorin (TECTA) gene, the p.Y1870C alteration in DFNA8/12 causes prelingual, nonsyndromic, autosomal dominant hearing loss. Here we investigated the effect of p.Y1870C on reverse transduction by audiometric studies in the family. Methods: Pure tone audiometry, brainstem evoked response audiometry, the Freiburger test for speech understanding and transient evoked and distortion product otoacoustic emissions were assessed in three available affected members bearing p.Y1870C. Results: Pure tone audiometry showed U-shaped curves with moderate to severe degrees of hearing impairment confirmed by brainstem evoked response audiometry. Transient evoked and distortion product otoacoustic emissions were completely absent in all affected family members whereas word recognition scores were up to 95%. Conclusions: Although the missense p.Y1870C TECTA mutation leads to complete failure of the cochlear amplifier in humans, very high speech perception scores can be achieved with appropriate therapy.

HINTERGRUND: Der Zweck dieser retrospektiven Studie war, die Inzidenz und klinische Signifikanz von Infektionen mit HPV (Humanes Papillomavirus) in Patienten mit Kopf-Hals-Tumoren, die eine Strahlentherapie erhalten hatten, zu evaluieren. PATIENTEN AND METHODEN: Bei 88 Patienten mit Karzinomen des Kopf-Hals-Bereiches wurde eine Untersuchung zur Identifizierung von high-risk HPV mittels Immunhistochemie, PCR (Polymerase Chain Reaction) und in-situ Hybridisierung durchgeführt. 26 Patienten hatten ein Karzinom der Mundh?hle, 45 des Oropharynx, sieben ein Larynx- und 10 ein Hypopharnyxkarzinom. 29 der 45 Patienten mit einem Plattenepithelkarzinom des Oropharynx erhielten entweder eine alleinige Strahlentherapie oder eine Kombination mit Cisplatin oder Cetuximab. ERGEBNISSE: Von den 29 untersuchten Patienten, die zur Therapie ihres Oropharynxkarzinoms eine konservative Therapie erhielten, hatten 11 Patienten einen HPV positiven und 18 einen HPV negativen Tumor. Den Patienten wurde eine Strahlentherapie ± Cisplatin oder Cetuximab verabreicht, wobei die Patienten mit einem HPV positiven Tumor ein signifikant besseres Ansprechen auf die Therapie zeigten (p = 0.015). Auch das krankheits-spezifische überleben war deutlich besser in HPV positiven Patienten (p = 0.001). SCHLUSSFOLGERUNG: Patienten mit einem Oropharynxkarzinom und einem positiven HPV Status sprechen deutlich besser auf eine Radiochemotherapie an als Patienten mit einem HPV negativen Tumor. Das HPV Screening ist eine sehr einfache Prozedur und kann einfach routinem?ssig in die Standard Operational Procedures inkludiert werden.

EINLEITUNG: Noch heute stellen große Schleimhautdefekte in der Mundhßhle eine Herausforderung in der Plastisch-Rekonstruktiven Chirurgie dar. Um funktionstüchtige orale Schleimhaut herstellen zu kßnnen, wird eine große Menge an Zellen mit einem sehr hohen proliferativen Potential benßtigt. Das Ziel dieser Studie war es, zwei Zellfraktionen aufgrund ihrer Affinität zu Kollagen Typ IV zu isolieren. METHODIK: Orale Keratinozyten wurden aus gesunder Schleimhaut isoliert und je nach Affinität zu Kollagen Typ IV in zwei Zellfraktionen unterteilt. Wachstumskurven, Western Blot Analysen und Immunhistochemische Färbungen wurden durchgeführt, um Unterschiede zwischen diesen beiden Zellpopulationen zu untersuchen. RESULTATE: Zellen, die innerhalb von 20 Minuten an Kollagen Typ IV adhärierten (RAC-IV), zeigten ein hßheres proliferatives Potential, eine signifikant hßhere Expression von Integrin β1 und eine geringere Apoptose Rate als jene Zellen, die kaum eine Adherenz an Kollagen Typ IV zeigten (RAC-IV-D). Die schnell adhärierenden Zellen zeigen einen typischen, den Keratinozyten entsprechenden polygonalen Zellaufbau. Hingegen die RAC-IV-D Zellen weisen eine irreguläre Zellstruktur auf. Immunhistochemisch konnten deutlich weniger apoptotische Zellen in der RAC-IV, als in der RACIV-D Fraktion nachgewiesen werden. Darüber hinaus war die Expression von β1-Integrin signifikant in den der RAC-IV-Zellen erhßht als in den RAC-IV-D Zellen (p < 0,05). SCHLUSSFOLGERUNGEN: Die Ergebnisse unserer Studie zeigen, dass es mßglich ist, orale Keratinozyten mit einem hohen proliferativen Potential durch ein einfaches und billiges Zellisolierprotokoll zu gewinnen.

The correlation between p16 upregulation and high-risk HPV infection is well known in head and neck squamous cell cancer. However, no information is available on rare carcinomas of the minor salivary glands. We analyzed 38 samples of minor salivary gland malignancies. p16 expression was determined by immunohistochemistry, HPV DNA by using in situ hybridization to detect low (type 6 and 11) and high (type 16 and 18) risk HPV types. In 71% of samples p16 expression was found. Low-risk HPV DNA could not be detected in any of the samples, whereas high-risk HPV DNA was found in two samples of mucoepidermoid carcinoma, which also showed the highest immunoreactivity with p16. HPV infection does not seem to be a common event in minor salivary gland carcinoma. Upregulation of p16 is present in the majority of cases but only diffuse staining correlates with HPV 16 and 18 infections.

? 2021 The AuthorsIntratympanically applied treatments are of increasing interest to the otologic community to treat sudden sensorineural hearing loss or vestibular disorders but also to deliver gene therapy agents, or biologics to the inner ear. Further diversion from the middle ear and perilymph to blood circulation and cerebrospinal fluid via the cochlear aqueduct are one of the limiting factors and so far not understood well enough. In this study, intratympanically applied triamcinolone acetonide was determined in cerebrospinal fluid. Additionally, perilymph was sampled through the round window membrane as well as at the lateral semicircular canal to determine drug levels. Of the twenty-one included patients, triamcinolone acetonide was quantifiable in cerebrospinal fluid in 43% at very low levels (range 0 ng/ml–6.2 ng/ml) which did not correlate with perilymph levels. Drug levels at the two different perilymph sampling sites were within a range of 13.5 ng/ml to 1180.0 ng/ml. Results suggest an equal distribution of triamcinolone acetonide to semicircular canals, which might support the use of triamcinolone acetonide as a treatment option for vestibular pathologies such as Menièrés disease. On the other hand, the distribution to cerebrospinal fluid might be limiting current approaches in gene therapy where a central distribution is unwanted.

EINLEITUNG: Die Tonsillektomie ist die h?ufigste Operation der Hals-, Nasen- und Ohrenheilkunde. Im Durchschnitt werden an der Universit?tsklinik für HNO in Wien etwa 500 Tonsillektomien pro Jahr durchgeführt. Bei 1,8 % der Patienten trat eine erhebliche Tonsillensp?tblutung auf. Diese Inzidenz stimmt mit der von anderen Autoren berichteten Rate von 2–4 % überein. Derzeit wird eine routinem??ige pr?operative Gerinnungsdiagnostik zur Pr?vention postoperativer Blutungen ohne Einzelfaktorenanalyse durchgeführt. MATERIAL UND METHODEN: Innerhalb des Jahres 2007 wurde in 3 Patienten mit pr?operativ erhobenen unauff?lligen Gerinnungsparametern aber rezidivierend auftretender Tonsillensp?tblutungen (2 bis 7 mal) ein kompletter Gerinnungsstatus durchgeführt. RESULTATE: Nach Auftreten von rezidivierenden Sp?tblutungen zeigte die Einzelfaktoranalyse einen Faktor XIII, einen Faktor XII Mangel sowie bei einem Patienten einen Willebrand Faktor Mangel. Nach Verabreichung eines Gerinnungsfaktorsubstituts konnten die rezidivierenden Sp?tblutungen gestoppt werden. SCHLUSSFOLGERUNG: Die Ergebnisse zeigen, da? die derzeit durchgeführte routinem??ige pr?operative Gerinnungsdiagnostik eine Lücke bei der Erfassung eines erh?hten Blutungsrisikos aufweist. Die Untersuchung von Einzelfaktoren bei Auftreten einer postoperativen Sp?tblutung trotz eines normalen pr?operativen Gerinnungsbefundes dient zur Diagnostik von versteckten Gerinnungsst?rungen, die eine spezifische Therapie erm?glichen. Die Einschlie?ung dieser Analysen in die pr?operative Diagnostik k?nnte daher auch das postoperative Blutungsrisiko senken.

Background It has been shown that birch pollen immunotherapy can induce IgG antibodies which enhance IgE binding to Bet v 1. We aimed to develop a serological assay to predict the development of antibodies which enhance IgE binding to Bet v 1 during immunotherapy. Methods In 18 patients treated by Bet v 1-fragment-specific immunotherapy, the effects of IgG antibodies specific for the fragments on the binding of IgE antibodies to Bet v 1 were measured by ELISA. Blocking and possible enhancing effects on IgE binding were compared with skin sensitivity to Bet v 1 after treatment. Results We found that fragment-specific IgG enhanced IgE binding to Bet v 1 in two patients who also showed an increase of skin sensitivity to Bet v 1. Conclusion Our results indicate that it may be possible to develop serological tests which predict the induction of unfavourable IgG antibodies enhancing the binding of IgE to Bet v 1 during immunotherapy.
